Please use this form to let us know if you encounter any problems with the program, feel like something is not working correctly, or have a recommendation on how we can make the program better.

If you’re reporting a bug, please include which version of the app you have (iPhone or Android), the nature of the problem or difficulty experienced (i.e., crash) and a description of the situation that led up to the issue, such as: which symptom guide was accessed, what tab you were on, etc.
