We have created this page in an effort to assist our patients and their family members in finding community resources that may help them maintain safety and improve/maintain quality of life. -Medical Records: Swedish is required by law to maintain the privacy of your health information, to provide you with a notice of its legal duties and privacy practices, and to follow the information practices that are described in the Privacy Notice on our main Swedish.org page.
Our Health Information Management Department can help you obtain your medical records.
You may want to authorize disclosure of your medical records for the purpose of:
• Verbal disclosure only to your family members/caregivers
• Request for continuing care release to a new physician

-Special Needs: If you are deaf, hard of hearing, deaf-blind or have limited English Proficiency, we may be able to assist you. The Office of the Education Ombudsman (OEO) resolves complaints, disputes, and problems between families and elementary and secondary public schools in all areas that affect student learning.
