About the Medical Home Model
Swedish Community Health Medical Home in Ballard will serve as Swedish's first pilot site that utilizes the Patient-Centered Medical Home Model. This is a model of care that comprehensively addresses the needs of patients through the following:
Personal Relationship
Each patient has an ongoing relationship with a personal physician and a health care team (nurses, counselors, social workers) who provide continuous and comprehensive care.
Team Approach
Each health care team, including the clinic support staff, takes responsibility for the patient's care.
Expanded Access
Access to care is available through scheduled and same-day appointments. Communication takes place between the patient, physician, health care team and support staff via e-mail and telephone. Patients also have secure access to their own health records online.
Comprehensive Care
Each health care team manages a broad range of health care needs for patients of all ages. Care is arranged with specialists when needed.
Coordination of Care
Care is coordinated with specialists and outside services to assure that the patient gets the best possible care.
Quality and Safety
The entire medical home team is involved in continuous quality improvement based on patient and staff input.
