The Collaborative Health Action Team (CHAT) is a unique health care delivery model to better serve individuals who are living with and managing chronic health conditions. A nurse care manager assigned to a client providing support, education and resources to help empower self management of their chronic condition in order to maintain an active lifestyle.
There are 2 types of programs supported by CHAT:
- Long term nurse care management for identified high risk clients from a select service provider.
- Short term nurse care management post discharge from a Swedish facility.
- CHF, AMI, PNA
