Advance Directives
Have you ever thought about what kind of health care you'd want if you became critically ill or injured? While it's not exactly something any of us wants to think about, it is so important to make these decisions beforehand so that your family is well aware of your requested wishes. The Swedish guide "Talk About It!" offers help on communicating your health-care wishes to others.
At Swedish, we encourage you to make informed medical treatment decisions ahead of time by using legally valid documents called Advance Directives. Make sure your physician, family members and friends have copies of your signed Advanced Directives and remember to bring copies to the hospital to be included in your chart.
Learn more via our Advance Directives brochure which includes two important printable documents: a Durable Power of Attorney and a Health-Care Directive (Living Will).
